COVID-19 Missing Person Notification Form

This system allows New Yorkers to file a report about a friend or loved one who may be missing due to COVID-19. Reports will be investigated by NYPD and should be made for missing people who are known to live in NYC.

Reports will only be accepted for individuals who are missing, and not known to be in specific hospitals. The missing person report will be sent to NYPD for further investigation.
