Homebound New Yorkers Can Get the COVID-19 Vaccine at Home
Note: The City's in-home vaccination program, which had been on a pause along with all Johnson & Johnson vaccine administration in New York City, will resume using the Moderna vaccine starting this week. Screening calls for verifying eligibility have resumed.
New York City is vaccinating City residents at home with the COVID-19 vaccine if they are fully homebound. "Fully homebound" means that the individual cannot leave their home, even with assistance.
Please fill out this form if you are homebound and interested in receiving the vaccine at home. Your completed form will be shared with the New York City Department of Health and Mental Hygiene. This form does not guarantee an appointment. You will receive a call to discuss the program and your eligibilty. It may take a few weeks to receive a call from the City.
